#2 is a dendrite.. but I'm not sure exactly what it's labeling. Perhaps axons vs cell bodies, etc.. but hard to say because those are specific growths in the areas of the letters, too. 'a' might just be the dendrite itself. ..or maybe #2 is a secret alien language and this human host is carrying the key to conquering Earth... |

It could be there to acknowledge the saying "Wearing your heart out on your sleeve".
Some of these are pretty cool, except where the tattoo artist obviously had little experience with art and drew like I did when I was in 4th grade.
