Brand new bottles (the old glass kind) were the worst ... and it still wasn't a big deal.
While tipping the bottle take a butter knife and put it in the bottle down the side and then move the knife toward the center and back out, creating a cavity/bubble of air on one side of the bottle ... ketchup will flow freely.

I hate the squeeze bottles in restaurants. The bottles are red so they never look "used" to the customer. They also never look like they need filling to the staff so I always end up having to wait while they refill the bottle, before I can eat a meal I want ketchup for.
