I used to work in fast food. For something like raw food, we'd refund the order, remake, plus probably provide something extra (desserts). If they didn`t bring it back, would still offer a replacement for another time. The whole equal return for refund only really applied to the chronic complainers.
There is a major difference between raw and incorrect. I expect incorrect from maccas, so I check the bag. I expect food past the hold time. I do not expect raw, and since that is a lawsuit waiting to happen, you do not let the customer walk away unhappy.
