Maybe she really was dead. The criteria necessary for declaring somebody dead have changed over the years, partly due to the pressure to harvest organs. People can now be declared dead while there's still brain activity or when their hearts are still beating. Or even when they`re still moving. Doctors are advised not to tell grieving relatives that their `dead` relative has moved to avoid upsetting them. Doctors are discouraged from carrying out some tests for fear that they might show that the patient is still alive. All this is according to the New Scientist but I can`t provide a link that doesn`t need a subscription to the magazine in order to read it.

Chick: "Hi. Why am I here?" Doc: "F*CK!!!" Chick: "What's wrong?" Doc: "I really needed that liver." Chick: "Well... You can have it. I don`t need it, anyway..." Doc: "Great!! If the incision is along here-" Nurse: "DOCTOR!!!" Doc: "Oh, fine! Send her back..."
