There's a really simple way to prevent this. Just make the weights much larger in diameter, then start them out in a slot so the bar is the same height as the current regulation. As soon as they clear the slot, close it up, so if they drop it, the bar stops at about 18-24" from the floor.

As a former high school and college weight lifter, this is why you always use spotters. I realize that most high level competitions don't use them, because it interferes with the judging, but this sort of thing illustrates why you always, always, always use spotters in the gym.

this is a bad post. that man almost died
He got up and walked away and most of the world has seen it over and over on breakdowns of the Olympics. I hardly think showing a pic internationally viewed accident that he walks away from safely constitutes a bad post.
