I am a cosmetic surgeon working with many cosmetic lasers everyday. For me the setup was quite simple: one balloon was inserted in a bigger balloon, filled with water, then the bigger one was blown, and both were tied. As for the laser, the particular laser used was of a wavelength that would be absorbed by water, not the balloons. The water would then boil and explode.
