These are all true to some degree except for the last two. I've never seen an occasion when someone was laying on the ground. That`s something for TV drama, I think. The deep bow one is funny though. I think only women do it like that. A guy would just bow low. It depends on the situation mostly. I think a foreigner who stays in Japan long enough would just pick up when and how to bow. Apologies, though, must always come with bowing that is the one sure fire rule. Japanese people don`t take your apologies as sincere if you don`t bow.

Yes, the majority of these are correct. For women, however, hands are to be placed in a prayer-like pose slightly above bust level. I was a foreign exchange student last year.. got a lot of rude looks until I learned all the customs.
