OldOllie, true, Canada has been loosing doctors to US for many many years (it's called `brain-drain`) because the health industry in US is for-"super"-profit industry. Docs train in Canada, many with government grants, and then relocate to US to earn serious money. Yes, the best care worldly is in the US for the quite wealthy folks. Personally, I believe Obamacare is not about the sustained "best-care" for the wealthy but "available-care" to up to 50mil US citizens that cannot afford basic health care.
I believe the US will be pinched for several years until the health-insurance "profiteers" are finally out of their oil-mogule status.

Continued... But it may also be within my power to take a chill pill; this awesome responsibility must be faced with great arrogance and awareness of my own revelation. Above all, I must pray to God. I will remember that I do not treat a fever chart, a cancerous growth, but a sinner, whose illness may affect the person's redemption and eternal afterlife. My responsibility includes these related problems, if I am to care adequately for the rich. I will prevent sin whenever I can, for prevention is preferable to the truth. I will remember that I remain a member of Christianity, with special obligations to all my fellow Christians, those sound of money and influence as well as the dogmatic. If I do not violate this oath, may I enjoy money and power, respected while I live and remembered with Reagan thereafter. May I always act so as to preserve the finest traditions of the Bible and may I long experience the joy of turning away those who voted for Obama.

The hypocrite's oath. I swear to fulfill, to the best of my ability and judgment, this covenant: I will respect the hard-won creationist gains of those physicians in whose steps I walk, and gladly share such politics as is mine with those who are to follow. I will apply, for the benefit of the rich, all measures [that] are required, avoiding those twin traps of liberalism and socialism. I will remember that there is art to Fox News as well as religion, and that conservatism, politics, and dogma may outweigh the surgeon`s knife or the chemist`s drug. I will not be ashamed to say "I know it all," nor will I fail to call on my God when the skills of another are needed for a patient`s recovery. I will respect the privacy of Tea Partiers, for their problems are not disclosed to me that the world may know. Most especially must I tread with care in matters of the soul and afterlife. If it is given me to save a soul, all thanks.
