Distraction. Probably were talking on their phone at the time.

I have to confess - I did this once: drove off with the nozzle still in the fill tube. Don`t remember the details, other than I was having a bad day, and obviously distracted (but not on the phone).

Good thing it had an emergency breakaway connector installed. in the hose. Made a pretty good noise when it popped loose, so I notice right away. No damage to anything, and no gas spilled. I was able to simply click the hose back together.
