He is actually most likely checking the elephant's cervix during pregnancy. Every pre-natal exam includes the doctor shoving a couple fingers (or, in larger mammal`s cases` an arm) in there to make sure the cervix hasn`t dilated more than it should at whatever point in the pregnancy the woman is. I worked at a zoo a while back and we had a couple pregnant animals at the time - this looks a lot like those exams.
