The spikes aren't easy to get in, but they are definitely possible. Most skin spikes use dermal anchors (a hook that attaches to the skin underneath with the decorate spike visible). To do this, an incision is made into the skin and the anchor pushed underneath. Then, it is set into place and left alone for healing. Others make an incision, squeeze the jewelry in the opening, and allow the skin to heal over the jewelry`s flat base. In other words, yes, it`s painful.
