Abstinence is never an option. I mean really who does that anymore? People stop holding on to your virginity like it`s special to meet that "special someone" because when that special someone drats you over, you are gonna wish you drated everything that moved before hand.

@weirdgrrl: Menopause fits with simim23`s point perfectly. If you are sexually active and don`t use birth control because you can no longer have kids, you would circle menopause. If you aren`t sexually active, you could put "abstinence" or "N/A" or "None" on the line.

@simim: Your semantics are correct, but (a) how do you explain their inclusion of "menopause" as a form of contraception and (b) sometimes this part of the form is mandatory to fill out, so what do you put if you`re not currently sexually active?

Since when has it ever been an option?

Birth control is something you use while sexually active or during sex to prevent having children.

Since you are abstaining from being sexually active, abstinence is not considered birth control.

Its not a resume, it looks a lot like the form girls fill out to go to a gyno. They ask about your job and stuff because stress or heavy lifting can affect our girl parts, especially when you are pregnant.

Why is it condom/diaphragm AND foam/gel? What about JUST condoms? And hysterectomy/menopause don`t seem like birth control to me. That`s just a byproduct of those things...

For those of you asking what kind of form asks about your employment and about your birth control methods, I would guess Health Insurance, since Health Insurance covers some forms of contraception and is dependent upon employment.
